Each business must have a valid certificate on file. Afterwards, Swire will retrieve data from Colorado web site to renew when expired. However, if there is any ownership, location or other major change to the business, a new certificate will be required.
• Fill out upper portion with business information.
• This is a multi-jurisdiction certificate. Please enter both state and city tax numbers associated with your business. Most home-ruled cities now require certificates showing their city tax number as well as state number.
• If you have additional locations covered by this certificate, please provide a list with physical addresses and city tax license numbers for each location.
• Must be signed by owner, Partner or Corporate Officer and dated to validate certificate.
